Lost/Replacement of Teaching Permit

 

As the Teaching Permit is given to the Academic Staff member for his own safe-keeping, he is required to ensure that it will be available for sighting whenever required by the University during accreditation/audit visits by the relevant bodies.

 

Should the Teaching Permit be lost or need to be replaced due to any reasons, the Academic Staff member would be required to forward the following documents to DHR for submission to MoHE.

 

1) Passport size photo (2 pcs)

2) for lost of teaching permit - police report or statutory declaration 

3) for replacement - statutory declaration & copy of the damaged teaching permit

4) Money Order/Postal Order/Bank Draft for RM10.00 (subject to current rate charged by MoHE).
